In addition to bringing an open mindset and a willingness to move, we ask that you bring your license or form of photo ID and your insurance card. You should bring or wear comfortable, athletic fitting clothing that will not restrict your movement during the examination or treatment.

Active Health Chiropractic is an out-of-network provider with all insurances, and may work with your plan to cover our services. We can check your individual plan for such coverage if you provide your full name, date of birth, insurance carrier and member ID. We will give you more information about potential coverage and any out of pocket costs after we verify with your plan.
